DESCRIPTION
Created a custom form on the demo instance of v2. The system was then upgraded on Azure. Applied the saved config after the upgrade and the custom form does not show in the Record Information form group where it was originally. Moving the form to another group - Survivor Assessment for example - and it does show as expected. Move back to Record Information and it does not show
